Listed below are several forms we use in our practice to
faciliate your medical care. If there is sufficient time
before your appointment, we can mail these to you or you
can download the forms to print, read and fill out ahead
of time or you may come early to your first appointment
with us to fill them out. Please click the links to download

Patient First Visit with Our Office
When you come for your first visit with our office, please bring a photo ID such as a driver’s
license, your insurance cards, compression hose you own that fit you if you have any and these
forms:
Patient Venous History Form - After Conservative Therapy
This form is used after a patient has completed a trial period of conservative therapy. Not for use at your
initial visit with us - we will give you this form to complete when you come after your Conservative
Therapy period is complete.
Patient Privacy
Our practice fully endorses a patient’s right to privacy for their medical care. Our policy is available for
review any time.
For Outside Physician Office Use Only - Ultrasound VUE Order Form
This form is for physician offices to use when ordering ultrasound tests on their patients. Patients can
not order their own ultrasound diagnostic tests.
Post Procedure Patient Instructions
After you have a procedure in our office, we will provide you with specific instructions for the care of your
body and review them with you to ensure you understand them and have no questions. Additional copies
of those instructions can be found here in case you need them.
